## About the person and the assessment

Who will assess me, and which board holds their licence?

How will you assess the problem I want help with?

What might lead you to suggest a different kind of care?

## About the treatment and its evidence

What is the exact treatment name?

What change do you expect for my goal?

Which source supports that change in people like me?

What does the evidence leave unanswered?

What other choices can we discuss?

## About the bill and the care afterward

What is included in the written cost?

What changes after an introductory offer?

What happens if I stop?

How do I reach you with questions after my visit?

What is the plan if a problem occurs?

## If medicine will be sent to you

Ask who will make and send it. Get the pharmacy’s full name before checking the record.

State boards of pharmacy license pharmacies; the Food and Drug Administration (FDA) directs readers to those boards.

FDA guidance checked on September 17, 2026, says to avoid an online pharmacy unless your state board’s records include it.

A listing shows a licence record; it does not tell you whether each prescription is filled properly.

Find the state pharmacy licence search

Read the FDA pharmacy-check note

Use the pharmacy named in the care plan or on the label when you search. Ask for an explanation if the details do not match.

FDA: find a licensed online pharmacy

## Carry this forward

Keep each answer with its source or written terms.

Ask again where an answer is unclear. Pause where an answer is missing.
